Who we are
Quartz Dental is a dental practice located at 188 Yorkville Avenue, Suite 400, Toronto, ON M5R 1C2. We are registered with the Royal College of Dental Surgeons of Ontario.
What information we collect
We collect personal health information necessary to provide dental care. This includes contact details, medical and dental history, insurance information, clinical findings, treatment records, radiographs, intraoral scans, and photographs taken for clinical purposes.
How we use it
Patient information is used solely for providing dental care, communicating with you about appointments and treatment, billing insurance providers when authorised, and meeting our regulatory obligations. We do not use patient information for marketing without explicit written consent.
Who sees it
Access is limited to the clinical and administrative staff involved in your care. We may share information with referral specialists when a referral is necessary, with insurance providers when you authorise direct billing, and with regulatory bodies when required by law. We do not sell, rent, or share patient information with third parties for any other purpose.
How we protect it
Patient records are stored in an encrypted practice management system with role-based access controls. Physical records and imaging archives are kept in locked, access-controlled areas. Our systems are audited annually against PHIPA and PIPEDA standards.
Your rights

Website analytics
Our website does not use third-party analytics or marketing trackers. Server logs record standard request data (IP, user agent, timestamp, URL) for security and operational purposes only, retained for 30 days.
